A CLINIC. DURING A PANDEMIC. WITHOUT BEING A TREATING CLINICIAN.

The Problem

"Most clinicians who become practice owners never make the shift that actually matters — from thinking like a clinician to thinking like an owner."

They build practices that are busy. Maybe even profitable. But entirely dependent on their constant presence — which means the moment they step back, everything slows down. They work harder than they expected, earn less than they deserve, and wonder why the freedom they imagined when they signed the paperwork never quite arrived. This isn't a failure of ambition. It's a failure of curriculum. Clinical training prepares you to deliver excellent care. It does not prepare you to build an organization, manage cash flow, hire for culture, read a P&L, negotiate with payers, or build toward an exit that actually rewards the risk you took. From Clinician to Owner closes that gap. Not with theory — with the frameworks, decisions, and hard numbers that separate thriving practices from ones that merely survive. Written by a clinician who has sat on every side of the table: OT, practice owner, acquirer, and consultant.

The practice worth selling and the practice worth owning are the same practice."

Building deliberate systems, developing a capable team, and reducing owner dependency doesn't just increase exit value — it creates the daily freedom that most practice owners wanted when they started but never quite managed to build.

What's Inside?

The Full Arc

Most practice business books are written by consultants who have studied healthcare from the outside. This one is written by someone who has done the job — and structured around the decisions you'll actually face, in the order you'll face them.

  • Part I The Mindset Shift: What actually changes when you become an owner — and why most clinicians stall out because they never make the mental transition from practitioner to builder.

  • Part II Buy vs. Build: An honest, numbers-grounded comparison of both paths in — including the deal Rafi made to acquire a struggling outpatient clinic during the pandemic and what he learned from it.

  • Part III Operations & Infrastructure: The systems manual nobody told you to build — scheduling, documentation, SOPs, and the operational skeleton that separates a thriving practice from a chaotic one.

  • Part V Positioning, Marketing & Growth: How to differentiate your practice in a crowded market, build referral relationships that compound, and use technology for growth rather than distraction.

  • Part VI The Financial Picture & Exit: Cash flow, payer relationships, and the financial metrics that actually matter. What your practice is worth, how it's valued, and how to build toward a number worth achieving.
